*There are two DHS forms, one for students/guardians and one for staff. The text for each of these forms can be different. Unless you want the forms to be identical, please submit separate text for each form.
**Only one failure threshold can be set for each subsection. The failure threshold determines how many symptoms or exposures within a subsection need to be checked to produce a “Not Approved” screening value. The failure threshold applies to BOTH THE STUDENT/GUARDIAN AND STAFF DHS FORMS.
